Directions: The National Junior Honor Society is not solely interested in your GPA and academic
accomplishments, but rather in you as a complete person. Please thoughtfully consider all sections and
provide any information that you believe the Faculty Council should take into account during
the selection process.

II. Leadership-----Are there situations in which you have taken on a leadership role (directing and
motivating others), such as ASB/Student Government, leadership, WEB, Scouts,
church/synagogue groups, team captain, etc? List all elected or appointed leadership positions
or other positions of responsibility. Please explain how you have been involved, and what this
leadership has meant to you.
The main situation where I took the role of leadership, was when I became track manager last school
year. Being involved as part of an organization made me feel mature and shows that I can be trusted. Itr
also helped me become more responsible as a young citizen.
III. Activities------- List all activities in which you have participated. These may include sports,
musical study, drama, after school work, etc. Please explain your involvement and
accomplishments in these areas.
I have participated in some activities. Delta-math, girl scouts and track(as manager) are some of the
few activities I took part in. In delta-math I didnt take
IV. Awards/Recognition-------Being invited to join the NJHS is an award in and of itself. Please
share any other awards or recognitions you have received that you would like for the Faculty
Council to know about. Please include when you received the listed awards or recognitions.
V. Community Service------ An important component of Honor Society is service in and for
our local and global communities. A minimum of 10 hours of community service this school
year is required in order to be considered for the NJHS. Service cannot be to immediate family
members.
Describe one of the ways you are thinking about how to earn your 10 hours of community
service this school year.
VI. National Junior Honor Society and You------Becoming a member of NJHS is both an
opportunity and commitment. Explain why you wish to be a member of the NJHS, what special
qualities you will bring into the membership and why you should be considered for
membership.
