1.

List the school activities you have participated in, the number of years, and the amount
of time per week, and what you have gained or learned from each activity.

I have been apart of several clubs while attending heritage my junior year. Key club,
Make A Wish, Relay For Life, and Student Government. We have a meeting every week
of the school year for each club. I have gained a lot from these clubs such as my
leadership abilities and learned to help others in our charity events. Teaming up with
others to help our school and community has helped me benefit to becoming a better
person.

2. Which activity has been most important to you and why?

Relay For Life has benefited me the most because I have participated in several charity
works that have helped so many people, and has changed lives of those less fortunate.

3. What do you consider your most important activities outside of school? List jobs,
volunteer work, religious activities, travel, and hobbies. Include the number of years of
your involvement and the amount of time spent on the activity weekly, and explain why
this activity was significant to you.

Playing baseball has been my most important activity; I have been playing on varsity
since my freshman year. It is my passion to play and we play and practice everyday
before the spring seasons starts. Baseball has brought many opportunities such as gaining
friends, traveling experiences to other states and baseball has just brought me pure joy
and excitement. Another important activity is my religious practice as a Christian; I
attend church every Sunday for 3 hours along with my family and sometimes friends.

4. If you work, how has this affected you?

I do not work because my parents believe that my job is for me to get good grades in
school and play baseball so it would hopefully get me into a great college.

5. In or out of school, which awards and honors have you received? Which elected
offices have you held? How do these achievements demonstrate your personal qualities?

The only awards I have got were the honor rolls in 9th, 10th and 11th grade, and varsity
awards of recognition for each season.

6. What book(s) have had the greatest impact on you? Why?

Apart from the bible, no books have really had an impact on me.
7. Describe the academic accomplishment (major paper, science experiment, artistic
project) you are most proud of, and tell why you take pride in it.

My accomplishment for my English class was I had to do my first research paper on the
book Ethan Frome. I never had to do a research paper for my old school so it was quite a
challenge. I take pride in it because I got an A and whenever I take upon a new challenge
such as a difficult paper, I always succeed.

8. What kind of learner are you (i.e. do you learn best in groups, from lecture, by doing?)
Which academic setting or assignments make you thrive? What interests you? Give
examples.

I learn best by listening. That’s the way i've always been learning by listening to people
talk, such as t.v, radio, and teacher’s lectures. As long as someone is always talking I am
paying attention and trying to pick up anything I need to know. In school I’d rather put
away the book and just listen to the teacher lecture because it just sticks in my mind, kind
of like photographic memory.

9. List three positive adjectives to describe you. Explain each and give an example that
demonstrates this quality.

Extroverted- My extroverted personality makes it easy for me to be comfortable in any

social situation because I’m so outgoing and always like to make friends. An example
would be that when I go to parties I usually tend to be center of attention and like making
the crowd I’m with laugh.
Competitive- All my life I have been the competitive in my family, always striving to be
the best that I can. Still to this day, I use this attribute in my baseball and school. It
always helps me to get further and better.
Quick learner- I have the cunning ability to just pick things up so fast, and just carry on
with anything. My quick learning skills have helped me with baseball, school lectures
and projects, basically anything that I need to learn fast.

10. What do you hope to accomplish in college and after. Consider your career goals and
your broader goals. It is OK to be broad and general if you do not have specific career
goals in mind.

All my life, my plan was to follow in my father and grandfather’s footsteps and pursue a
career in business. Anything that will make me money, to become successful and buy
whatever I want, has always been my exterior motive. In college I want to pursue a
certain degree in whatever takes me towards my goal. Besides that I hope to play college
baseball and play all my 4 years, and to be drafted so I can go on to play professional
baseball, that has always been my dream.
11. Do you believe your transcript is an accurate representation of your ability and your
readiness for college? If no, explain why.

Yes, because everything that you do is recorded and put into documentation, which
reflects everything that you do in school, such as SAT, ACT, and your GPA. All your
actions are put on paper, and that tells whether you’re ready for college or not. There’s
not better way but a transcript to show your hard work and work ethic.

12. What are your strengths? How do you use them?

I believe in myself; self-confidence, I use them with everything I do in life, from school
to hanging out with friends to baseball. Whenever someone tells me i'm cocky, I just
reply no im confident. Without confidence, it’s hard to get anywhere in life, and I plan to
use it to get me anywhere that I want to go.

13. What are your weaknesses? How do you overcome them?

I used to have trouble with procrastinating, now I have learned to write down a list of
things that I need to do, and keep a calendar to keep track of deadlines. I have found that
this not only helps me to finish things on time, but it has also helped me to be more
organized.

14. How would your family describe you to an admissions officer (it is ok to ask them)?

They would say “My son is a funny, charming kid who always strives to be at the top.
He’s determined to be successful in life, and he cares deeply for his family and friends.”
