referred to a birthing facility & will be attended by skilled health
professionals

mothers will undergo 4 ANC & 2 postpartum visits
-Participation of Barangay Health Committee through the BHB in
maternal death review & infant mortality review will be advocated

Mortality to atleast 15/1000 Live
Births and under Five Moratlity to -Provides advocacy to attain 70% of all infant EBF up to six

-Identification of clients with unmet Modern Family Planning in the


assigned barangay so with outreach medical services will be
conducted

-Modern contraceptive method among WRA to atleast 65% in the

-Strategies to decrease adolescent birth rate to 2% per year will
be advocated
-Assistance on Family Program services and the prevention of
teenage pregnancy in the assigned barangay will be provided

-100% advocacy that residents will be Evacuated to assigned


4 .Reduction of Mortality rate due to
Evacuation Areas and participate in data gathering response
injuries
during health emergencies and disaster.
