AFFIDAVIT OF INVESTIGATOR
I, SPO3 CHRISTALLYN J TOTOL, a bonafide member of the Philippine
National Police and presently assigned at IBA MUNICIPAL POLICE STATION as
Investigator with office address at Brgy. Lipay Dingin Panibuatan, Iba, Zambales,
after having been duly sworn to in accordance with law do hereby depose and state,
That, I am presently detailed at IBA MUNICIPAL POLICE STATION (IbaMPS) as
Station Drug Enforcement Unit (SDEU) Investigator.
1) That on November 19, 2023 at about 02:10 PM, joint elements of Iba
Municipal Station and Provincial Drug Enforcement Unit (PDEU) operatives
led by led by SPO3 CHRISTALLYN JOSE TOTOL (Lead Unit), Zambales Pr
Provincial Drug Enforcement Unit (PDEU) led by PMAJ RICARDO LOPEZ
conducted Anti-Illegal Drugs Operation (Buy Bust) at Purok 2 Bagong
Kalsada, Brgy. Palanginan this municipality covered with PDEA coordination
control number (10005-111923-0558) relayed by DUTY ROC: Jacob
Manglicmot which resulted in the arrest of the suspects namely; Suspect
Brian Carino y Ursua, male, 21 years old, (D/POB: October 25, 2002-Palauig,
Zambales), single, vocational graduate, jobless and resident of Dampay
Salaza, Palauig Zambales (Watchlisted) after the poseur buyer bought one (1)
piece of plastic sachet containing chrystalline substance that is suspected as
shabu from the suspect.
2) Confiscated pieces of evidence were the following:
a. One (1) pc. Heat sealed transparent plastic sachet of suspected shabu
(buy-bust item);
b. Three (3) pcs. Heat sealed transparent plastic sachet of suspected shabu
(in possession of the suspect.
c. One (1) pc. Five Hundred Peso Bill (Php500.00) bearing serial number
AA444888 (buy-bust money);
d. One (1) pc. unit of black cellphone Redmi Note 10 pro.
e. One (1) plane black sling bag.
That, in compliance to Section 21 of RA 9165, the inventory, markings and
photographs of pieces of evidence confiscated/recovered was conducted at the
place of commission and started at around 03:32 PM of November 19, 2023.
Wherein, all pieces of evidence were turned over by the Arresting Officer, SPO1
Lycel L Lozada to me as Investigator-On-Case at the place of operation in the

presence of Hon. Lucifer Gwapito (Elected Brgy. Chairman) and the
abovementioned the arrested suspect. The inventory ended around 04:30 PM of
same date. After which, suspects and pieces of evidence brought the Iba MPS for
documentation and proper disposition. Thereafter, all drug specimens were properly
handled, stored and safeguarded personally by the undersigned prior to its turn over
to Zambales Provincial Forensic Unit for laboratory examinations, and upon arrival at
Zambales Provincial Forensic Unit, all drug specimens received by Duty in Forensic
Unit PMAJ Marion G Domingo as mandated by law and jurisprudence.
That, we maintained the proper documentations and chain of custody of the
confiscated pieces of evidence for its integrity and reliability and also after the
inventory I gave copies of inventory receipt to the witnesses (Elected barangay
Chairman of Palanginan) and the arrested suspects upon arrival at Iba Municipal
Police Station.
That, we know, we need to videotape (record) our operation but since we do not
have an issue body worn camera, we will use alternative recording device (cell
phone) to record the operation so we need to get an ex parte motion but due to the
suddenness of the information we received and if we will apply ex parte motion, we
may lose the opportunity to catch the said target so we decided to pursue the
operation without ex parte motion.
That the undersigned was designated as investigator on case of said operation
and have caused the preparation of the following documents;

That, I assisted the affiants in preparing their joint Affidavit of Sworn
Statements;
That, I am executing this affidavit to attest to the truthfulness of the foregoing
statements and support the filling of appropriate charges against the suspects;
