School Year 2022-2023, the undersigned applicant who is a holder of professional license from the
Professional Regulation Commission as a teacher hereby respectfully submits his/her written application
supported by the following documents:

1. CSC Form 212 (revised 2017) in two copies with the latest 2x2 picture ID
2. Certified Photocopy of PRC professional identification card
3. Certified Photocopy of ratings obtained in the Licensure Examination for Teachers/
Professional Board Examination for Teachers;
4. Certified copy of Certified Authentication Verification of Transcript of record or Authenticated
true copy of Transcript of Records for baccalaureate degree and post graduate degree;
5. Copies of Service Record/ Certificate of employment, performance rating, and school
clearance for those with teaching experience;
6. Certificates of specialized training, if any, like computer literacy, journalism, theatre arts,
National Certificate (NC I or NC II) ,etc;
7. Certified copy of Voter’s ID and / or any proof of residency as deemed acceptable by the
School Screening Committee
8. NBI Clearance

The undersigned applicant assumes full responsibility and accountability on the validity and authenticity
of the foregoing documents submitted and commits that any violation will automatically disqualify the
undersigned applicant from the selection process.

I believe that my instructional abilities acquired from my formal education, combines with my years of

My formal education includes Bachelor of Elementary Education major in General Education. At


present, I am a Locally Hired Teacher assigned at Leganes Central Elementary School. I am very familiar
with the leadership responsibilities with this profession and I perform such other functions as may
assign from time to time. I utilize many of my skills and also had the opportunity to work closely in the
school and as well in the District Office of Leganes. One of my duties was to help school head and
teachers in preparing documents and other paper works in school. I was also assigned as a subject
teacher of Grade 4 class. In this way, I had been able to share my skills and knowledge in teaching
during limited face-to-face classes. The opportunity to work as an educator was the most exciting
prospect for me, and I’d love to play a role in contributing learning development to my learners.
