The major maintenance and housekeeping activities performed by our personnel in our
classroom are: 1. maintenance: computer, printers, scanner, aircon; 2. Housekeeping:
cleaning tables and chairs, cleaning floor, walls, windows, ceilings, ventilation,
illumination.

What procedure do you follow to request for the purchase of additional tools,
equipment and machines for your classroom? What improvement would you
recommend for this procedure?
The purchase request is prepared by the section head of the work area where items to
purchase is located. Then, this request is approved by the department manager. The
accountability is very clear. The only thing to improve is more details in the
specifications. Also, the vendor must be a selection of three.

What is the SOP of your institution when equipment is found to be out of order? Would
you recommend an improvement to this procedure? Why or why not?
When an equipment is not working, I will contact a repairman. In the meantime, I will put
appropriate tag-out labels. Yellow, if caution. Red, if warning.

Describe your institution’s waste disposal system. If none, what can you recommend?
Our waste disposal system is simple. A waste can for recyclable, a separate waste can
for composting, and another waste can for disposing.

Describe your institution’s recording and documentation of equipment/machine
maintenance repair. If none, what can you recommend?

A Breakdown/Repair Report is filed when there is an equipment that mal-functions. This
document states the findings, the recommendations, and the action taken. The trainer
will prepare the breakdown/repair report. Then, the Work Request is prepared. It is
prepared by the trainer addressed to the department head. If the repair does not work, a
Salvage Report is prepared. A history of each equipment is recorded.
