IMPORTANT REMINDERS:
1. Print and bring the accomplished application form;
2. Bring 4 pieces of identical ID Pictures, Passport size (with name tag with signature over printed name (First name, Middle Initial, Last name, Ext. name (if any)));
3. Bring Original and Photocopy of valid I.D cards;
4. For applicants without date of birth in the I.D cards, bring original and photocopy of birth certificate issued by PSA or Local Civil Registry;
5. For applicants holding dual citizenship, bring original and photocopy of Identification Certificate (IC) issued by the Bureau of Immigration; and
6. Prepare Examination fee of Php500 for the CSE-PPT.
